I dati di base sono presentati nella Tabella 1. Le correlazioni tra età eeGFR calcolatoutilizzando le 3 equazioni erano significative (p<0.001). The R2 of the eGFR/Jm-CKD-EPI (R2 =0.509) was larger than that of the eGFR/Jm-eGFR (R2 =0.150) and eGFR/Jm-MDRD (R2 = 0.083).

In particular, regarding the relationship with older age (>70 years old), the area under the ROC curve (AUROC) for the eGFR/JmCKD-EPI was much larger, (0.859) than that for the eGFR/ Jm-eGFR (0.674) and eGFR/Jm-MDRD (0.636). Fig. 3b shows the results of the ROC analysis between the eGFR calculated using the 3 equations and the 5 comorbidities, excluding an older age (>70 anni), (n=798). Discussione
Dei tre eGFR, l’eGFR/Jm-CKD-EPI correlava in modo più sensibile con le comorbilità. eGFR/Jm-CKD-EPI, eGFR/Jm-eGFR e eGFR/Jm-MDRD hanno rilevato differenze significative rispettivamente in quattro, tre e una delle cinque comorbidità (Tabella 2). Compared to donations from healthy living donors, those from living donors with medical conditions (so-called expanded criteria donors) exhibited a high incidence of overall and death-censored graft loss according to multivariable Cox proportional hazards analyses (hazard ratios=2.16 and 3.25, p= 0.015 and 0.004, respectively) (7).

Negli studi trasversali, un eGFR/CKD-EPI più basso ha mostrato una migliore associazione con la prevalenza di comorbilità rispetto a eGFR/MDRD nelle popolazioni residenti in comunità caucasiche. Tarantini et al. (4) hanno riferito che nei pazienti con CVD, la prevalenza di CVD era più elevata quando si valutava eGFR/CKD-EPI rispetto a quando si valutava eGFR/MDRD nel gruppo con eGFR basso. Juutilainen et al. (13) hanno valutato i tassi di comorbilità, vale a dire ipertensione, obesità, diabete e CVD, in pazienti con insufficienza renale cronica e hanno osservato una prevalenza significativamente più elevata di pazienti con comorbilità quando la valutazione è stata eseguita utilizzando eGFR/CKD-EPI rispetto a quando è stata eseguita utilizzando eGFR/MDRD.
Abbiamo confermato l’accuratezza dell’eGFR/Jm-CKD-EPI in letteratura. Regola et al. (14) hanno riferito che l’equazione CKD-EPI era più accurata della MDRD nelle popolazioni a basso rischio, compresi i donatori di rene pre-donazione e post-donazione. Murata et al. (15) hanno riferito che l’eGFR/CKD-EPI basato sulla creatinina ha dimostrato meno bias rispetto all’eGFR/MDRD nei potenziali donatori di LKT (-8% vs. -18%). Burballa et al. (16) e Gaillard et al. (17) hanno confrontato i valori di eGFR/CKD-EPI, eGFR/MDRD e mGFR basati sulla creatinina con gli isotopi nei donatori LKT preoperatori e hanno concluso che eGFR/CKD-EPI correlava meglio con mGFR rispetto a eGFR/MDRD. Horio et al. (18) hanno confrontato l’accuratezza di eGFR/Jm-CKD-EPI e eGFR/Jm-MDRD con il GFR dell’inulina misurato in una popolazione sottoposta a controllo sanitario in Giappone. Horio et al. (19) evaluated the accuracy of the eGFR/Jm-eGFR in potential LKT donors in Japan who received the inulin clearance test and observed a bias (mGFReGFR) of 18.3±16.4 mL/min/1.73 m2. Thus, the eGFR/JmeGFR underestimated the true GFR of LKT donors. Based on the two studies of Horio et al. (18, 19), the eGFR/JmCKD-EPI appears accurate for comparing measured inulin GFR values.
We explored why the eGFR/Jm-CKD-EPI was superior regarding its relationship with the five evaluated comorbidties, as the reasons have not been examined in-depth in previous reports. The comorbidity rates were 2 to 3 times higher in the elderly group (age >70 years old) than in the non-elderly group (age 70 years old) among the LKT donors (Fig. 3). An ROC analysis revealed that the eGFR/JmCKD-EPI was better associated with older age (>70 anni) rispetto a eGFR/Jm-eGFR e eGFR/Jm MDRD (Fig. 3a). Riteniamo quindi che l'equazione caratteristica della sensibilità all'età sia responsabile della superiorità dell'eGFR/Jm-CKD-EPI.
Ji et al. hanno studiato la relazione tra eGFR e danno preclinico agli organi bersaglio nell'ipertensione utilizzando un'analisi ROC e hanno riferito che l'equazione eGFR/CKD-EPI cinese era meglio associata a complicanze ipertensive rispetto alle equazioni eGFR/MDRD cinese e asiatica modificata (20). L’eGFR/CKD-EPI era meglio associato rispetto all’eGFR/MDRD con lo spessore intramediale, l’indice caviglia-braccio, l’indice di massa ventricolare sinistro, il rapporto albumina/creatinina urinaria e la velocità dell’onda del polso aortico. Nel nostro studio, escludendo i casi anziani, eGFR/CKD-EPI era meglio associato a comorbilità nei donatori LKT rispetto a eGFR/Jm-eGFR e eGFR/Jm-MDRD (Fig. 3b). Pertanto, l’eGFR/CKD-EPI potrebbe essere sensibile alle complicanze ipertensive o aterosclerotiche, escludendo il fattore età avanzata. L’eGFR/Jm-CKD-EPI è raccomandato per l’uso nelle valutazioni del rischio, non solo per il danno renale ma anche per il danno d’organo sistemico, riflettendo le complicanze ipertensive nei donatori di LKT.
È stato riportato che eGFR/CKD-EPI è superiore a eGFR/MDRD nella previsione di eventi CVD o mortalità nei partecipanti caucasici (1-3). Nei partecipanti cinesi, l’eGFR/CKD-EPI era un migliore predittore di recidiva e morte di ictus rispetto all’eGFR/MDRD (21). Consistent, Matsushita, et al. (22) hanno riferito che l'eGFR/Jm CKD-EPI era un migliore predittore del rischio di mortalità per tutte le cause e cardiovascolare rispetto all'eGFR/Jm-MDRD nell'intervallo dell'eGFR 60 mL/min/1,73 m2 nei partecipanti giapponesi. Terawaki et al. (3) hanno utilizzato un'analisi ROC per confrontare i valori predittivi di CVD e ictus tra eGFR/Jm-CKD-EPI ed eGFR/Jm-MDRD e hanno riferito che gli AUROC per eventi CVD nell'eGFR/Jm-CKD-EPI ed eGFR /Jm-eGFR erano 0,596 e 0,562, rispettivamente. L’eGFR/CKD-EPI era più strettamente associato all’incidenza di CVD in 241.159 partecipanti giapponesi (età media 64 anni) sottoposti a un controllo sanitario generale. Ohsawa et al. (23) hanno riportato una previsione migliore per mortalità per tutte le cause, infarto miocardico e ictus con l’eGFR/Jm-CKD-EPI rispetto all’eGFR/Jm-MDRD in una coorte di controlli sanitari. Pertanto, l’eGFR/CKD-EPI è superiore nel predire gli eventi CVD e la mortalità nelle popolazioni che vivono in comunità. Dovremmo monitorare attentamente i donatori con un basso eGFR/CKD-EPI dopo il trapianto.

Several limitations associated with the present study warrant mention. The registry data had no data on the measured GFR, so we could not directly compare the accuracy of the three eGFR equations. We unfortunately had to exclude many cases with missing data from the analysis. These limitations might have resulted in the data being misclassified; however, our study has some important insights derived from its involvement of a large cohort of LKT donors (>8,000 casi).
Conclusione
L’eGFR/Jm-CKD-EPI era meglio associato a comorbilità, tra cui obesità, ipertensione, diabete, CVD e ictus rispetto all’eGFR/Jm-eGFR e all’eGFR/Jm MDRD nelle popolazioni a basso rischio, come i donatori LKT giapponesi. Per la valutazione iniziale della funzionalità renale dei candidati donatori LKT, si raccomanda l’eGFR/Jm-CKD-EPI, in particolare per i donatori con criteri estesi con comorbidità.
